Do you need a form because you have no e-Identification or a registered home address?

For those without E-identification but with a registered home address

You can order your COVID certificate by printing out, filling in, and posting this form. The certificate will then be mailed to your home address as recorded in the Swedish population register. Currently the turnaround time when you order the certificate this way is about two weeks. If you have e-Identification, and access to a computer, tablet or mobile phone, we request that you use www.covidbevis.se

Please note that a reserve number (temporary medical number) cannot be used.

For those without a registered home address

If you have a coordination number (samordningsnummer) and your vaccinations are registered in the National vaccination register, you can request a vaccination certificate by printing, filling out and posting this specific form (see the instruction below).

If you don’t have a registered home address in the Swedish population register, you can fill in the address that you want the certificate to be sent to. The Swedish eHealth Agency will send the certificate by registered mail to a service point. You will get a notification from PostNord when you can pick up your letter.

Please note that a reserve number (temporary medical number) cannot be used.

Instructions for using the forms

  1. For those without E-identification, but with a registered home address: Print out and fill in this form. (Remember to sign it!)
  2. For those without a registered adress in the Swedish population register: Print out and fill in this form. (Remember to sign it!)
  3. Post your form, in an envelope, to:

    391 29 Kalmar

    (Note that you cannot send the form, or your vaccination information, via e-mail. Your completed form must be sent by post.)
     
  4. We request your data from the National vaccination register, then send your COVID certificate, in paper form, to your home address as recorded in the Swedish population register, if you have one.

    You who don’t have a registered home address in the Swedish population register will get your vaccination certificate sent as registered mail to the address you have given on the form that you filled in (item 2, above). 

Legal trustees ordering a COVID certificate for their wards

If possible, wards should provide their signature.

In the event that a ward is unable to provide a signature, the document may be signed by the ward’s trustee.

Then mail the form, and attachment, to the Swedish eHealth Agency, together with a copy of a population register extract that verifies your trusteeship of the individual in question.

Unable to print out the form?

If you don't have your own printer:

  1. Get help from a family member, a relative or a friend with a printer.
  2. Visit a service center (servicekontor för myndighetskontakter) and print the form. Find a service center (in Swedish)

  3. Visit a citizen's office (kommunalt medborgarkontor) and print the form.

  4. Ask in a library if you can print the form there.

If you cannot use any of the ways above, please send an e-mail to covidbevis@ehalsomyndigheten.se and clearly state whether you need the form for those without e-Identification or if you need the form for people without a registered home address in the Swedish population register.

When you receive the form, follow the instruction above and fill it in, sign it and send in the form.
