If you do not have an e-identification and/or registered address
If you do not have an e-identification, you can visit one of Sweden's approximately 1,400 pharmacies and get help with your vaccination certificate there. You can also order your vaccination certificate by printing, filling in and mailing a form. If you have an e-identification and access to a computer, we ask that you use the service www.covidbevis.se
For those who do not have an e-identification but have a registered address
You can order your vaccination certificate by printing, filling in and mailing the form “Ordering a vaccination certificate”. The certificate is sent to your registered address. If the form is filled in correctly and the vaccine doses are registered, it takes a maximum of two weeks to receive a vaccination certificate.
If you have an e-identification and access to a computer, tablet or mobile, we ask that you use the service www.covidbevis.se. Please note that so-called reserve numbers cannot be used.
For those who do not have a registered address
If you have a coordination number and your vaccinations are registered in the National Vaccination Register, you can request a vaccination certificate by printing, filling in and posting the form "Ordering a vaccination certificate for those who do not have a registered address".
If you do not have a registered address, fill in the address where you would like the certificate sent. The Swedish e-Health Agency sends the certificate as a registered letter to an agent. You will receive a notification when your certificate has arrived at the agent. Please note that so-called reserve numbers cannot be used.
How to use the forms
- Print the correct form and fill in the requested information. Remember to sign the form.
- Put the form in an envelope and mail it to us.

(Keep in mind that you cannot e-mail the form or information about your vaccinations. The form must be sent to us by post.)
- We request information from the National Vaccination Register and then send a paper copy of your Covid certificate to your registered address (if you have one).
If you do not have a registered address, your vaccination certificate will be sent by registered letter to the address you have provided on the special form for those without a registered address.
Legal representatives who want to order a Covid certificate for a principal
If possible, the principal must sign with his or her own signature.
If the principal is unable to sign his or her own name, the document must be signed by the principal's legal representative.
Then send the form to the Swedish e-Health Agency together with a copy of the register extract that proves that you are a legal representative for the person.
